We are currently recruiting a Senior Embedded Case Worker to support timely discharge of patients within the Mental Health hospitals in Oxford.
About the Service
Working in partnership with Oxford Health NHS Foundation Trust, our service supports patients on mental health wards at the Warneford and Littlemore hospitals. We help people overcome practical barriers to discharge, ensuring they can return to safe, suitable accommodation and access the support they need to recover and maintain their independence.
The Impact You Will Have
As a Senior Embedded Case Worker, you will work closely with hospital teams and patients to address the practical issues that can delay discharge or increase the risk of readmission. You will provide hands-on, person-centred support, helping people prepare for life back in the community and ensuring they have the right support, resources, and living environment in place before they leave hospital.
Key responsibilities include:
- Supporting patients to maintain their accommodation and reduce the risk of homelessness.
- Helping people address practical barriers to discharge, including housing, benefits, utilities, identification, and household essentials.
- Working alongside patients, hospital staff, family members, landlords, and partner agencies to coordinate support and achieve positive outcomes.
- Supporting people to access grants, debt advice, health and wellbeing services, and other community resources.
- Assisting with resettlement tasks such as sourcing furniture, white goods, food, clothing, and other essentials.
- Referring clients to appropriate services for longer-term support following discharge.
- Maintaining accurate records and monitoring outcomes in line with service requirements.

Contract: Permanent, subject to funding
Salary: £28,541 - £30,367 per annum, dependent on experience
Hours: 37.5 hours per week, Monday – Friday.

About you
We're looking for someone who is compassionate, resourceful, and committed to helping people achieve positive outcomes. You will enjoy working directly with people, building trusting relationships, and finding practical solutions to the challenges they face.
You'll bring:
- Experience of working with people experiencing mental health challenges, homelessness, housing issues, or other complex support needs.
- The ability to build positive relationships and work in a person-centred, strengths-based way.
- A practical and proactive approach to problem-solving, with a willingness to get involved and support people with a wide range of issues.
- Experience of supporting people to access services, benefits, housing, or other community resources.
- Good organisational skills and the ability to manage a varied workload in a busy environment.
- Experience of maintaining accurate records and using IT systems, including Microsoft Office applications.
