We are seeking a compassionate & experienced Mental Health Nurse to join our evolving team of placed based homeless workers. This unique & bespoke role,
In partnership with CGL offers the opportunity to deliver trauma-informed, person-centered care to individuals facing homelessness, mental health challenges, & substance misuse.
You will work closely with the Coventry Clinical Lead & CGL to bridge gaps in service provision & improve outcomes for this highly marginalized population who face significant health inequalities & barriers to care. This is a patient facing community role.
Key Responsibilities
- Provide mental health support to adults who are rough sleeping, homeless, or at risk of homelessness.
- Facilitate access to drug/alcohol services/support & mental health services.
- Collaborate with voluntary & statutory services, including physical health & outreach teams.
- Jointly assess & support individuals with dual diagnosis in partnership with CGL.
- Work flexibly & autonomously, this may include on occasions outside st&ard hours, to meet client needs.
- Advocating for service users, reducing disengagement, & helping them navigate services.
- Providing responsive clinical input, including crisis management & harm reduction in community settings.
- Actively contributing to multi-disciplinary team (MDT) meetings to ensure joined-up care.
