What You’ll be doing 

Are you passionate about helping people live independently and stay connected to their communities? As a Reablement Support Worker, you’ll join a supportive multi-disciplinary team, providing person-centred support that helps individuals build confidence, develop their independence and achieve their goals.

You’ll be part of a responsive, customer-led service operating seven days a week, working closely with individuals to understand what matters to them and provide the right support at the right time. Your work will make a real difference in helping people maintain their independence and continue living safely and confidently in their own homes and communities.

 What we’re looking for

As a Reablement Support Worker, you’ll provide person-centred support that helps individuals build confidence, maintain their independence and remain living safely in their own homes. You’ll work closely with individuals, carers, families and professional colleagues, using your communication skills and initiative to identify and respond to concerns.

You’ll support people in line with their care and support plans, including carrying out therapeutic activities that promote independence and wellbeing. You’ll need a good understanding of reablement, rehabilitation and equality, along with the ability to keep accurate records using IT and electronic monitoring systems.

You’ll need to be able to undertake manual handling tasks and hold, or be willing to obtain within 12 months of appointment, an NVQ Level 3 in Health and Social Care or equivalent. A full, valid driving licence and access to your own vehicle are essential, as you’ll travel across the Borough. You’ll also need to be flexible and willing to work a rota covering evenings, weekends and some bank holidays.

Because of the nature of this job, it will be necessary for the appropriate level of criminal record disclosure to be undertaken. This post is exempt from the Rehabilitation of Offenders Act 1974 and therefore applicants are required to declare all unspent cautions and convictions; and also any adult cautions (simple or conditional), and spent convictions that are not protected as defined by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (as amended in 2020). A person’s criminal record will not in itself prevent a person from being appointed to this post. However in the event of the employment being taken up, any failure to disclose such offence, as detailed above, will result in dismissal or disciplinary action by the Authority.
