We're looking for a compassionate, proactive and resilient Outreach Worker to join our Worcester & South Worcestershire team. This role is all about building trusted relationships with people sleeping rough or at risk of returning to the streets, helping them navigate services, secure accommodation and move towards a more stable future.

Location: Worcester (covering Malvern Hills & Wychavon)
Salary: £25,291.44 – £29,932.71 per annum (Band 1)
Hours: Full Time – 37 hours per week
Contract: 12 months FTC

About the Role

The successful candidate will deliver both outreach and inreach support, working intensively with individuals who are disengaged from statutory services and who require structured support to access the housing pathway and progress toward safe, sustainable accommodation.

This role works collaboratively with local authority Housing Options teams, outreach services, harm reduction partners and community agencies to ensure coordinated, person-centred support.

Main Duties of the Role

Housing Pathway & Casework

  • Support clients who are rough sleeping into temporary or settled accommodation through assertive outreach and structured casework.

  • Work closely with Housing Pathway Officers from Wychavon DC and Malvern Hills DC, plus the Rough Sleeper Coordinator.

  • Support clients to engage with accommodation options where no suitable alternatives exist.

  • Identify and address risks that may cause clients to lose accommodation; attend Platform Housing drop-ins fortnightly.

  • Refer clients to tenancy support services where appropriate so the Navigation focus remains on active rough sleepers.

Priority Groups & Targeted Outreach

  • Provide proactive support to Target Priority clients and people who are long-term or entrenched rough sleepers.

  • Joint work with council outreach teams and local agencies, including for high-risk visits or where lone working is unsuitable.

  • Attend Maggs Malvern Day Centre to engage new rough sleepers or those not engaging with Outreach/Pathway teams.

Meaningful Activity & Support Planning

  • Support clients to access meaningful activity, community reintegration, volunteering and health interventions.

  • Facilitate referrals to non-accommodation services (social prescribers, charities, volunteering programmes).

  • Facilitate referrals to statutory services including Adult Social Care, Mental Health, GP services, DWP and substance misuse services.

Monitoring & Reporting

  • Maintain accurate and timely records in accordance with Maggs’ case management requirements.

  • Support line manager to provide data required for quarterly monitoring to Wychavon DC and Malvern Hills DC.

  • Contribute to twice yearly progress summaries, including outcomes, risks, and mitigations.

  • Assist with delivering operational targets, supporting up to 10 clients per year as required by the Rough Sleeper Action Plan.
