The postholder will build trusting relationships with individuals, identify needs and risks, provide practical and emotional support and work alongside partner organisations to help prevent homelessness, reduce repeat presentations and support people towards greater stability and independence.

- Build trusting and respectful relationships with people who may experience barriers to accessing or engaging with statutory services.
- Identify individual needs, strengths and risks and work collaboratively with people to agree appropriate support and next steps.
- Provide practical and emotional one-to-one support around housing, homelessness prevention, health and wellbeing, benefits, budgeting, daily living and tenancy sustainment.
- Support individuals to access accommodation, healthcare, benefits, specialist services and wider community support through appropriate referrals, advocacy and warm handovers.
- Hold and manage an appropriate caseload, providing ongoing support to individuals where required and reviewing progress towards agreed outcomes.
- Work proactively with people at risk of homelessness to help prevent crisis, eviction, repeat homelessness and avoidable use of emergency or statutory services.
- Maintain accurate and timely case records, support plans, risk assessments, referrals and outcome information in accordance with Maggs’ policies and procedures.
- Work closely with local authorities, housing providers, health and social care services, voluntary and community organisations and other partners to provide coordinated support.
- Contribute to multi-agency working and coordinated case management where this will improve outcomes for individuals.

- Support community-based activities, drop-ins and other opportunities which promote wellbeing, connection and engagement.
- Encourage meaningful involvement and co-production, ensuring that people who use Maggs’ services and volunteers have opportunities to contribute to the development and delivery of the service.
- Work positively alongside volunteers and support their involvement within the Hub and wider community activity.
- Contribute to monitoring, evaluation and reporting by accurately recording activity, referrals, engagement and outcomes.
