Thames Reach runs projects and services that help to end homelessness in and around London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…
We are looking for a Lead Worker to join the Staying Well Team.
The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.
We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
You will:
- Work collaboratively with other departments to ensure high‑quality, holistic care for service users
- Create SMART goals, support plans, and risk assessments in partnership with service users
- Focus on engagement, social inclusion, autonomy, independence, and improved physical health outcomes
- Deliver a mixture of short‑ and long‑term interventions based on assessed need
- Supervise the practice, learning, and development of Trainees within the team
Support the Lead Manager with referral screening and initial client assessments
You will be:
- Confident working autonomously in the community with people experiencing mental health difficulties
- Organised, able to manage conflicting priorities and deadlines, including administrative tasks
- Proficient in Microsoft Office applications, including databases
- Confident in managing Trainees, offering clear guidance, constructive feedback, and opportunities for growth
- Committed to developing and sustaining effective working partnerships with a range of stakeholders
- Passionate about mental health and motivated to make a positive impact on the lives of people experiencing both short‑ and long‑
