ABOUT THE ROLE
We are seeking a dedicated Housing Navigator to join our team in Brent. You will provide intensive housing management support to our participants who are or at high risk of homeless and require additional support with sustaining their tenancy. You will be liaising closely with partner charities who specialise in housing support.
As a Housing Navigator, you will provide personalised, trauma-informed support to aid recovery, rehabilitation, and reablement in a psychologically informed environment (PIE). You will work closely with residents, managing a caseload that may include complex housing management issues, and collaborate with community partners to achieve positive outcomes. This role requires a compassionate and proactive individual who can thrive in a fast-paced, dynamic environment while maintaining the highest standards of service delivery.

Rota: 37.5 Hours per week, 08.00am - 16.00pm, 09.00am - 17.00pm or 10.00am - 18.00pm, Monday to Friday 1 day from the office, 1 day at the Brent Hubs and 3 days visiting participants in their home or community settings in and around Brent.
ABOUT THE SERVICE
Synergy, Brent is a Housing Related Support (HRS) Service for a range of vulnerable people who have been or are at risk of homelessness. The age eligibility criteria for the service is adults between the ages of 18 to 60, who are identified to have low/medium support needs. Some of the needs we support are:
- Single homeless between ages of 18 to 60
- Socially excluded individuals
- Ex-Offenders
- People with substance misuse
- Mental Health (Low)
- Learning disability (Low/Medium)
- Women who have come from domestic abuse environments and situations who may need low level support as part of their resettlement following a move on from a refuge or other safe domestic abuse supported accommodation.
ABOUT YOU
We’re looking for an individual who is driven to provide high quality care and support to others, ideally with previous experience in providing housing support. You will be someone who thrives on working in a team, but can also make individual judgements in a proactive manner. You will be confident in working in the given setting and can be compassionate, supportive, and empowering to others with the ability to form effective, positive, and motivational relationships! We work alongside various partners and stakeholders so we are looking for a Support Worker who can adapt and work well alongside other teams.
What we are looking for:
- IT Proficiency, ability to learn new software programs, basic Microsoft experience
- Previous experience and knowledge within a supported housing management setting
- Experience creating co-produced support plans and providing appropriate interventions for residents/participants and liaising with other professionals
- Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents

What we would like, but not essential:
- Qualification: NVQ/Diploma Level 2 in Health and Social Care/Community Justice/Psychology or professional equivalent
- Previous experience in a similar organisation/industry/role
- Understanding of the housing and social needs of people with multiple and complex needs

OVERVIEW OF KEY RESPONSIBILITIES
Various Property and Housing Management related responsibilities including:
- Provide advice and support on all aspects of tenancy management such as housing benefits, welfare benefits, appeals, and overpayments.
- Ensure properties meet contractual agreements with landlords and housing associations.
- Act as a primary support worker for a caseload of participants with different needs.
- Carry out regular health and safety checks, ensure all repairs and maintenance issues are correctly reported and managed through to completion.
Service Delivery:
- Provide face to face support, and contribute to the development of support plans, risk assessments, and reviews to a high standard.
- Create and implement SMART support plans in collaboration with participants.
- Participate in and encourage participants involvement in various projects, activities, and initiatives.
- Support the service and team to meet key KPIs and objectives.
- Recognise support needs in an individual and initiate appropriate interventions to prevent further deterioration.
- Complete safeguarding and other referrals where required.
- Ensure all participants understand their rights and responsibilities and have access to the right tools and resources, to support them back into society and building networks.
- Provide advice, information, guidance and life skills training to participants.
