Overview
Serveco is seeking a dedicated Mental Health and Harm Reduction Support Worker to work in partnership with Gravesham Borough Council to join our team, addressing the complex needs of the homeless population in the Gravesham area of Kent. In this rewarding role, you will engage directly with rough sleepers and vulnerable individuals, helping to reduce harm associated with substance use while providing essential mental health support. You will play a key role in building trust, promoting safer practices, and supporting clients on their journey toward stability, improved wellbeing, and accommodation.
Hours of Work: 37.5 hours per week Monday to Friday (may occasionally require evening and weekends)
Location: Gravesham
Contract: Fixed term (8 months)
PRINCIPAL RESPONSIBILITIES:
- Engage with individuals experiencing homelessness, building trusting relationships and understanding their needs
- Deliver harm reduction interventions, including advice, education, and distribution of supplies
- Conduct mental health assessments and provide emotional support, referring to specialist services where needed
- Carry out regular outreach (minimum two days per week) to verify and support rough sleepers
- Develop support plans, risk assessments, and move-on plans to help clients progress into accommodation
- Support individuals in maintaining tenancies once housed
- Work collaboratively with local agencies, shelters, and community partners to improve service access
- Advocate for clients to ensure they receive appropriate care and support
- Organise and attend outreach events, awareness sessions, and training initiatives
- Maintain accurate, confidential records of all client interactions
- Contribute to the development and continuous improvement of the service
Person Specification
Essential:
- Degree in Social Work, Psychology, or a related field
- Experience in harm reduction, mental health support, or homelessness outreach
- Strong interpersonal and communication skills, with the ability to engage hard-to-reach individuals
- A resilient, empathetic, and non-judgemental approach
- Ability to manage complex caseloads and work independently in community settings
Desirable:
- Knowledge of local services and support networks in the Gravesham area
- Experience working within a multi-agency or multidisciplinary team
