37 Hours per week, potential for hybrid working.

This post is wholly focussed on enabling successful move on; with the right support to sustain a tenancy, manage health and access services independently including social activities.
Working closely with our Move On Worker, the Transitional Worker will;
· Provide proactive, person-centred support to clients who have recently moved on from the hostel into the community
· Work closely with the Move-On Worker to ensure continuity of support and a smooth transition into independent living
· Build strong, trusting relationships with clients to encourage early communication of any concerns or challenges
· Conduct regular home visits to anticipate, identify, and resolve emerging issues early, preventing risks to tenancy sustainment, wellbeing, and safety
· Support clients to understand and manage their tenancy responsibilities, intervening early where difficulties arise
· Assist with budgeting, managing bills, and maintaining financial stability to prevent arrears or debt
· Help clients set up and sustain utilities, benefits, and essential services, ensuring stability from the outset
· Identify early warning signs of tenancy breakdown or disengagement and implement timely, preventative interventions
· Support clients to access healthcare, mental health services, substance misuse support, employment, and education, reducing risk factors linked to homelessness
· Encourage positive community integration and reduce social isolation to strengthen long-term stability
· Work collaboratively with landlords, housing providers, and partner agencies to resolve issues before they escalate
· Maintain accurate, timely records of support provided, risks identified, and preventative actions taken
· Safeguard clients by recognising and responding to risks in line with organisational policies

Pay: £32,000.00-£33,500.00 per year
