Harm Reduction Case Worker (Part-Time)
Location: Strand Road, Derry/Londonderry
Hours: 27 hours per week
Contract: Permanent
Working Pattern: Rota-based (mornings, evenings & weekends)
The Role
We are recruiting for a Part-Time Harm Reduction Case Worker to join a multi-disciplinary team delivering holistic, housing-led support services in Derry/Londonderry.
This role operates within a low-threshold, harm reduction framework, supporting individuals experiencing homelessness or at risk of homelessness. The successful candidate will provide high-quality, person-centred support aimed at improving housing stability, health, wellbeing, and overall quality of life.
You will work proactively to build trusting relationships with service users while delivering structured interventions, support planning, advocacy, and outreach work.
Key Responsibilities
Service User Support
- Deliver person-led, professional and respectful support.
- Assess service users’ safety and wellbeing needs, identifying and reducing risks.
- Develop and review individualised support plans in partnership with service users.
- Provide effective key working and targeted interventions.
- Support service users to integrate into their communities.
- Advocate on behalf of service users to achieve positive outcomes.
- Deliver outreach support within the community as required.
Health & Safety
- Ensure compliance with organisational Health & Safety policies.
- Maintain service security and risk management procedures.
- Complete required wellbeing and safety checks.
Administration & Record Keeping
- Maintain accurate client records and reports.
- Manage incident reporting, financial transactions, and internal/external documentation.
- Comply with IT and data security policies.
Team Contribution
- Participate in handovers, team meetings and training.
- Support volunteers and colleagues as required.
- Maintain professional standards aligned with organisational values.
Skills & Experience
- Knowledge of homelessness and related risk factors.
- Experience of needs assessment, key working and support planning.
- Understanding of statutory and voluntary sector services.
- Strong risk assessment and risk management knowledge.
- Competent IT skills and accurate report writing ability.
- Strong written and verbal communication skills.
- Ability to build professional relationships and network effectively.
Additional Requirements
- Ability to work on a 7-day rota including mornings, evenings and weekends.
- For outreach roles: access to a car insured for business use and a valid driving licence.
