Job Description:
WeCare Bespoke are looking for support workers to provide high-quality, person-centred support to young people who are homeless or at risk of homelessness, substance misuse and Mental Health issues. This role involves working with young people to build the skills, confidence, and resilience they need to move towards independent living and a more stable future.
Key Responsibilities:
- Build positive, trusting relationships with young people, offering emotional support and practical advice.
- Conduct assessments to identify individual needs, risks, and aspirations.
- Develop and regularly review support plans tailored to each young person’s goals.
- Support young people with housing, education, employment, training, health, and life skills.
- Liaise with external agencies (e.g. housing providers, social services, mental health teams) to advocate for and coordinate services.
- Assist with tenancy sustainment, helping young people understand their rights and responsibilities.
- Respond appropriately to safeguarding concerns and follow organizational safeguarding procedures.
- Maintain accurate and up-to-date records using case management systems.
- Encourage participation in activities that promote well-being, inclusion, and independence.
- Contribute to a safe and supportive environment within the accommodation or service setting.
Person Specification:
Essential:
- Experience working with young people or vulnerable individuals.
- Knowledge of issues related to youth homelessness, mental health, and substance misuse.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Understanding of safeguarding principles and confidentiality.
- Flexibility to work shifts, including evenings or weekends if required.
- Basic IT skills for record-keeping and communication.
Desirable:
- NVQ 2 Health and Social Care
- Experience working in supported accommodation or Homeless outreach settings.
- Naloxone Training
