POSITION: AIMS Family Mentor
LOCATION: Flint
HOURS: 40 hours per week
SALARY: £25,001
Main purpose of the role:
To provide a high quality and person-centred floating support service to Flintshire families who are or have experienced homelessness/ at risk of contract failure, by the delivery of targeted support packages, to empower families and build resilience to enable them to maintain contracts
Core responsibilities of the role will be:
- Delivery of a floating support service, conducting continuous assessments to identify the support needs of the clients and to formulate appropriate person-centred support packages
- To effectively manage case load and own time management
- To have up to date knowledge of current legislation and policies to ensure information being provided to clients is appropriate and accurate
- To deliver family housing, safety and tenancy sustainment skills in a way that empowers and promotes families to build resilience to manage accommodation effectively
- To signpost to relevant partner agencies
- To work collaboratively with other partner agencies
- To communicate safeguarding issues to the relevant colleagues and statutory bodies, where appropriate
- Identifying and record risks and formulate appropriate risk management plans, cascade any known risks or concerns to relevant persons
- To ensure use of good practice in risk management and lone working
- Responding to crisis intervention situations
- You will be based in our office in Flint however a full driver's licence and own vehicle is required
General Skills and Experience:
- A knowledge & understanding of providing support, advice and advocacy to vulnerable clients and the ability to communicate this knowledge
- Experience of working with people with complex needs
- Experience in delivering person centred support within similar setting
- Maintain appropriate written and electronic case management records
- To be aware of and adhere to all Local Solutions policies
- To carry out other duties appropriate to this post, as necessary or as requested
Specific Qualifications/Experience:
- Experience of supporting families who are or have experienced homelessness or are at risk of becoming homeless to successfully sustain contracts.
- Qualification in Youth and Community work, social work, HNC in Housing, Social Care NVQ L3 or equivalent
