Engage Leeds is a city wide housing support consortium led by Gipsil. We are looking for a Hub Support Worker to join our small, friendly team to process and triage referrals to the service, offer initial / short-term advice and respond to stakeholder queries. The role is a mixture of office and home working, so you must be able to manage your time effectively and be able to use your own initiative. This is a fast paced role that requires excellent organisational skills as well as a great eye for detail. As we are the first point of contact to referrers and people with support needs, we are looking for someone who has an excellent telephone manner and is able to handle various queries and conversations with confidence and empathy.
The main responsibilities of the post include: -
- Overall, to support the delivery of the Engage Leeds city wide housing support service.
- To support the effective functioning of the Hub - the central point of access for referrals and assessments.
- To process and follow up referrals delegated by the Senior Hub Co-ordinator and Hub Team Leader – Supporting people to develop independence and have greater choice / control.
- To deliver triage interventions and signposting for people being referred and for referring agencies, with a particular focus on prevention and early intervention
- To undertake needs and risk assessments where appropriate to ensure safe practice in referral, triage and community based interventions.
- To support and advise regarding housing support issues, including welfare benefits, accommodation options and money advice.
- To work with Hub colleagues and the Operations Manager to meet referral and assessment target timeframes.
- To liaise with colleagues across Engage Leeds ensuring the provision of high quality and timely support and advice.
- To update and maintain accurate records within the Gateway case management system.
