Teqniq is searching for a Supported Pathway Officer to work in the public sector.

36 hours per week.

12 week contract.

Job Description:

The Supported Pathways Officer provides person-centred support to vulnerable adults who have a history of homelessness or rough sleeping. The role focuses on supporting residents to move on into long term stable accommodation, while preventing homelessness.

The candidate will provide assertive client centred key working to assist residents to move along the housing pathway to independent living by providing support to maintaining their current accommodation and links into services and meaningful activities to improve their physical and mental wellbeing and life skills in order to prepare for and secure their next stage housing.

The candidate will provide resettlement support to residents as they move on, so that they successfully move into and sustain their new home.

The candidate will interview and risk assess new residents, carry out Risk Management and Safeguarding of Vulnerable Adult assessments for all new residents and induct them to the Health and Safety requirements of the accommodation and identifying whether a Personal Evacuation plan will be required.

The candidate will ensure intensive support is provided to help residents move into independent accommodation, with a clear pathway for housing identified with the resident.

The candidate will contribute to the day to day delivery of the hostel service, in accordance with all agreed objectives, procedures and policies, ensure required health and safety inspections, drills, room checks and records are carried out, that highlighted follow up work is undertaken, and that imminent dangers are made safe.

Working Model:

This is a frontline service working a shift pattern of early 8am – 4pm, late 12pm – 8pm, Monday – Friday no weekend s or bank holidays.

Role requirements:

Experience of working with homeless or vulnerable adults with support needs.

Experience of risk assessment, support planning and key working.

Ability to work collaboratively with safeguarding, health, social care and housing.

Ability to support residents to achieve positive outcomes and move towards independent living.
