Stepping Stones Project is a pathway to an independent living homeless shelter. The project houses the homeless and people threatened with homelessness. We offer one-to-one support and guide them to the areas that they need support. The aim of the project is to help move tenants from supported housing towards independent living. Our amazing support workers help with this journey.
PERSON SPECIFICATION (SUPPORT WORKER)

Job description
As a Support Worker, you will be part of a team of support staff who are responsible for ensuring all current and newly referred individuals receive high-quality, person-centered support. You will ensure good working practices Key to the role of a support worker is adopting a proactive approach to build a positive and engaging relationship with Local Authorities, GPs, Mental Health Workers, and any other agencies.
You will ensure the well-being of tenants and promote their development. A key component of this role is to assist tenants in their physical, social, emotional, and daily life skills development. This will then increase their independence and allow them to function appropriately in the community.
Service provided includes crisis intervention, behaviour management, preparing support plans on the information provided by the tenant, life skills training, whether this is on a one-to-one basis or in a group environment with other tenants in their accommodation.
1. To assist your tenants at home or in the community to allow them to function more safely and, independently.
2. Identify social, economic, recreational, and educational services in the community that will meet the tenant's needs. Maintain and liaise with other agencies, professionals HCC officials, and the community.
3. Recognise potential crisis/emergency situations, analyze such situations accurately and develop strategies to deal with them. The support worker must inform the Safeguarding Team or agencies which will be of appropriateness including the Manager/ Director.
4. Monitor the tenant’s wellbeing. Assist tenants either on a daily or weekly basis of life skills and or behaviour management training. Assist tenants to the hospital or, other appointments when necessary.
5. It is the responsibility of the support worker to ensure that all requirements are met to provide safe transportation i.e. proper driver’s license, roadworthy vehicles, and appropriate insurance (Business) cover.
6. Provide emotional support to the tenants and feedback to tenants and to their family and friends where concerned.
7. Maintain excellent written reports (logs) regarding tenants’ daily activities and progress, issues, and actions. Ensure that all required documentation is complete and accurate. These documents can be called up in a court of law as evidence.
8. Participate in the planning and help of the development of tenant’s support plan which will focus on the special needs of each tenant in a person-centered way. Outlining their goals and achievements since coming to the project.
9. Review and evaluate the support plans and evaluate your tenant’s achievements and make adjustments as and when required. Provide feedback and support to fellow staff and tenants. Support Plans are to be reviewed every 6 months, or when new information is provided
10. Encourage your tenants to join activities in the community and direct them to the office, where they will find an array of books, puzzles, etc.
11. Provide and liaison with other agencies in a professional manner
12. Provide input to management about the development of the support packages and participate in evaluations
13. Operate office equipment such as personal computers, photocopies, faxes, and printers, etc.
14. Answer phones and/or in-person inquiries and route to the following staff member if required. Respond to routine queries with regard to the organisation and services provided.
15. To be able to work on your own initiative and to be able to plan and organise your workload
16. Maintain the tenant’s confidentiality at all times
17. To take personal responsibility for your tenants and, to learn about each tenant’s condition/syndrome
18. Produce /contribute to assessments and report as required, attend and contribute to house meetings and any reviews that may be required by other organisations.
19. Attend and participate in all meetings as required, including own Supervision and Appraisals.
20. Work part of a team, share responsibility fairly, and be supportive of others.
21. Maintain an up to date knowledge, including attending courses, or online courses
22. Adhere to the no smoking policy, policy on the use of mobile telephone, and code of conduct policy on drugs and alcohol.
23. Adhering to all safeguarding requirements and any procedures aimed at the protection of vulnerable adults
24. Perform other related duties when required.
