Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…
We are looking for a Lead Worker to join the Hounslow Reach team.
Floating Support services work with people to sustain their housing. We use a strength-based approach to work with people to identify what support they require and how we can address them together or / and in collaboration with others e.g. landlords, Department of Work and pension or mental health services.
We can support people for the duration of their tenancy, or we can provide short term interventions. We can provide resettlement support when a tenancy commences, and we can equip people with the skills to manage their home and goals independently or when they are move on ready and require minimal support.
You will:
- Work with people who have spent time in hospital to assist them in achieving good outcomes, developing skills and confidence, raising their aspirations and reducing social isolation through connecting with their local community.
- Develop productive working partnerships with clinicians, social services, housing and community partners to source options and outcomes for people leaving hospital.
- Reflect, record and report on your work for colleagues and commissioners in order to develop services.
- Work assertively, with initiative, to problem solve, navigate a variety of systems and access services.
- Will in a respectful, trauma informed and respectful manner.
- Work professionally within a team.
To succeed as the Lead Worker you will have:
- Experience of empowering people to achieve the aspirational outcomes and build independence.
- Creative and proactive problem solving skills.
- Experience of being a positive and effective team member, achieve shared goals.
- Experience of developing and sustaining effective working partnerships with a range of stakeholders, in particular health and housing.
- Organisational and administrative skills, able to manage conflicting priorities and deadlines, communicate well, use a variety of IT systems, record your day-to-day work and reflect in writing on your work.
- Confidence, in particular to work autonomously within the community, in people’s homes and hospitals.
