An exciting opportunity has arisen for an Enhancing Lives Co-ordinator, within the Harm Reduction Team of Substance Misuse Services. The role will cover both Conwy and Denbighshire.
To oversee the management of the complex cases of people that the Enhancing Lives Team are working to support. Working directly with the caseload of people and the wider Enhancing Lives Team to develop and support the planning of what interventions are needed from the various parts of the system.
Job Summary/Job Purpose:
To be a senior member of the Enhancing Lives Team.
To work as the Co-ordinator for the Enhancing Lives Team; leading and supervising the team in taking a joined up approach to managing a caseload of people who are experiencing complex needs in relation to issues including substance use, homelessness, mental health and offending behaviour, which is often underpinned by trauma.
To lead and manage the implementation of health and social interventions to achieve the outcomes of engaging/re-engaging individuals that are experiencing complex needs, providing an integrated approach for service access.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
