Outreach Support Worker
We are seeking a highly motivated and empathetic Outreach Support Worker to join our team. The successful candidate will play a crucial role in connecting with homeless individuals and families, offering support to help them transition from temporary accommodation to secure and independent tenancies. The Outreach Support Worker will engage with clients, assess their needs, and provide guidance and resources to facilitate their journey towards stability.
Key Responsibilities:
Outreach and Engagement:
- Conduct regular outreach to identify and connect with homeless individuals and families.
- Build and maintain positive relationships with clients, establishing trust and rapport.
Assessment and Planning:
- Assess clients’ needs and goals to develop individualised support plans.
- Collaborate with clients to set realistic objectives for achieving sustainable, independent tenancies.
Advocacy and Liaison:
- Advocate on behalf of clients with external agencies, including housing authorities, social services, and healthcare providers.
- Establish and maintain effective communication channels with relevant stakeholders.
Resource Coordination:
- Assist clients in accessing essential services such as housing support, employment assistance, and educational opportunities.
- Provide information on available community resources and support networks.
Crisis Intervention:
- Respond promptly to crises and emergencies, providing emotional support and practical assistance.
- Work collaboratively with team members to develop crisis intervention plans when necessary.
Required Knowledge:
Data Protection:
- Confident in applying data protection and confidentiality principles to complex work situations.
- Contributes to the development of best practice and has a comprehensive understanding of individual and organisational rights, roles, and responsibilities related to data protection.

- Knowledge of local resources and services for homeless individuals and families.
- Commitment to maintaining confidentiality and upholding ethical standards.
- Understanding of substance and alcohol use, including harm reduction strategies.
Desired Qualifications & Knowledge:
- Hold or be willing to work towards an NVQ Level 3 qualification in Health and Social Care.

Pay: £24,000.00-£24,570.00 per year
